The facility was clean and the staff friendly. I would recommend them to others - they were dedicated to memory care only so the staff were experienced dealing with dementia patients.

This is a very clean and well maintained facility. The staff was very approachable and caring. The problem is it is a for profit facility,which the cost is roughly 5000.00 and up. They charge extra for everything. Diabetic meals,extra help with bathing,etc. Which means when her money ran out she would have to leave as they do not accept Medicaid or public assistance .

Very nice staff, but they need more!

Many times we have visited my father in law, and he has been walking around without his glasses. He needs his glasses. He is a fall risk. There have also been many instances where he was walking without his cane, and he should not have been. They need more helpers for the nurses. Dad has a roommate. It was less money. He receives extra services that we pay for. It all adds up.

Ortega Gardens Memory Care

They were great when moved in. They have steadily declined. They let the good CNA's go instead of paying them better and consistency with being with the residents. Many times there is only 1 person on the floor. It takes 2 people to help take them to the bathroom and put to bed and sometimes shower. They have meds tech on floor but you can't be watching the floor when trying to give medicine. It is unsafe. I wish I had a better report. We were happy when we first moved in, but there are just too many falls with no one watching.

Disappointing experience

My dad was diagnosed with Frontotemporal dementia 3 years ago. We wanted to keep him home as long as we could, but eventually, we couldn’t manage him at home. We spent months visiting facilities to determine which would provide the best care from him. We chose Mount Pleasant Gardens, because we were sold on a family feel and quality care. We are giving 2 stars, because some of the employees and the activities director/assistants are outstanding. The activities people are trying hard to provide a variety of activities for the residents. We knew that no place would be perfect, but we did expect a certain standard of care. In our 7 months at Mt Pleasant Gardens, we saw many things that caused us concern in the level of care being provided. Overall, there is a lack of professionalism and respect for the residents and their families. I’m addition, it seems that many employees are not properly trained on how to care for residents with dementia and mobility issues. Often during our visits, employees were sitting on their phones and didn’t even acknowledge us being there. We asked ourselves-if they aren’t attentive when we are here, how bad is it when we aren’t? The strict visiting hours concerned us as well. We were made to believe we could only visit during visiting hours, and were not treated well if we came early or stayed late. We now know that we legally have a right to visit 24 hours per day. We brought several concerns to management, but never felt they were truly addressed. We lost confidence in the managements’ ability to ensure quality care and we felt we had no other option but to move our dad to another facility. We are very pleased with the new facility and have seen a huge improvement in Dad’s mobility and attitude. We have chosen not to provide a laundry list of concerns on online review sites, but we have addressed those concerns to the upper management of Mount Pleasant Gardens, and hope that they will be working to improve the experience so that other families and residents don’t have similar experiences.

Not as it always seems

Our initial experience with Arbor San Jose started well enough, however as my fathers demensia progressed to where more direct care was required, he was refused to be accepted back following a hospital stay, despite being told specifically they could care for him even if he wasn’t mobile. We felt like they were shopping for patients that require the least amount of direct care resulting in our having to move him a couple more times with little notice and or options. My recommendation is to ask pointed questions.

Aggressive Behavior

Ponte Vedra Gardens advertises they care for dementia patients that suffer from aggressive behavior, which is a symptom of dementia, and the way they do it is by evicting a dementia patient that shows signs of being aggressive. After eviction it was determined that aggressive behavior can be controlled with proper medication.

Awful facility. BEWARE

They are thieves! Everything my mom took over for him would get stolen. She had his name on all of his belongings, clothes, toiletries, shoes, bed linens, towels, and ALL of it got stolen several times. He would be wearing other people's clothing, shoes, glasses, and when my mom asked about it, no one knew a thing! They stole his nice blankets, his ball caps! Disgusting. He had to go to the hospital a few times and each time the doctors said his medicine wasn't given to him correctly. They would make zombie like then send him to the hospital. When it came to eating, no one would help him. He is blind and almost deaf and only a few teeth. My mom would go visit during lunch or dinner and he would have food everywhere because he couldn't cut it or see it. His dentures went missing also and no one knew anything. That's an awful place. If we could afford it, we'd hire a lawyer. The monthly payment is over [removed] a month.

I would not recomnend this facility to anyone’s family member. They have frequent staff changes and the cleanliness of the facility is unacceptable. They have very few activities offered on weekends. Items of patients are frequently missing as well. Contract agreements are also not honored. You must visit frequently at random times if you have a family member at this facility.
