The staff is very welcoming and knowledgeable. They were also very caring and understand based on our difficult circumstance. It turned out to be a very smooth transition for us and our family member that moved to the facility. Over the first few weeks I received many updates on our family member. It was wonderful to know how great she was doing. [Removed] are truly wonderful at what they do!

Very caring staff and clean surroundings. I just wished I moved my mom here sooner while she was still able to enjoy all that they offer in activities. The staff is top notch and trained well. Would recommend to anyone wishing to place a loved one with memory problems.

* Bellevue is everthing Mom & I hoped it would be. They even welcome pets! * There are a number of floor plans to choose from, including 2 bedroom-2 bath, 2 bedroom-1 bath, 1 bedroom-1 bath, & studio. * Residents who transfer from independent to assisted status may stay in the same unit. * Bellevue assigns volunteer resident "ambassadors" to new residents, who take them to meals & activities & introduce them to others. * The monthly fee is quite high, but it includes everything, although that might not be ideal for folks who still want to do their own cooking. * Guests may dine for $6 per meal, which is added to the monthly bill. * It is a large community. Mom has to walk quite a ways down hallways to get to elevator, dining room, mail box, front desk, laundry, storage unit. In her case the walking is good for her. * Activities include 30-minute daily chair exercises which are excellent.

We are happy with the care or my dad. The community is clean and the staff is great. He is please with the menu and the variety. He is offered to do allot of activities that are offered to him. We would recommend the care and value of this community to other families.

We had a wonderful tour, they went through all the details and this is a beautiful facility.

Mom stayed here while she was recovering and needed some extra time before returning to her community. The staff were excellent. Mom did not like the food, however since she was in the memory care they did not offer a large variety of options. They offered a lot of activities and encouraged the residents to socialize. They have a central courtyard in this lockdown unit for the residents to get out and enjoy.

Our family is stilled amazed that we were able to find such an fabulous home for our parents. We have 5 siblings in 5 different states and with the help of "A Place for Mom" were were able to assess our options in Green Bay, WI from various states. Our parents are very happy in their new home at Bellevue.

i really liked the community when I toured. Every individual staff here are very good, it was nice meeting them. This facility was my number 2 choice. The room I saw was suitable with a private shower. The staff are skilled and they had activities available for the resident to enjoy.

This community is outstanding in all ways. the food is good. The activities are wonderful and helpful. I would highly recommend this community to other families for great care for a loved one.

My husband likes the care given here. The staff is very nice and helpful. the community is nice, but needs a little more care. Activities are always offered. We would recommend this for other families.

There is parking available for the people that reside there. This was one of many amenities offered by the community. There are meal plans available, as well as various activities. The tour guide showed me all around the bedrooms, the dinning area, and activities room. I thought the guide was very informative and answered my questions.

My sister is doing pretty well here at this community. She is still adjusting, but i know that is to be expected. They have been super helpful to me. They are great with informing of anything that happens. It is a well maintained community, everything is very nice and clean. I know she is being cared for well, and I would recommend this community.

This is a great place! The people working here; nurse, administrators, therapists, activity people and caregivers are all friendly, kind and caring. The food is terrific also!

We are very happy with the care and the staff are good people to interact with. My mom is liking the activities they provide. She has not complained about the meals, so I am assuming she likes it. The community is definitely clean in every area. This community is specifically for memory care and it is going well for us.

Autumn Leaves is lovely. New, and you can tell. Beautiful appointments. Extremely secure. Just for the memory-impaired. Not exactly right for my Mom at this time, because she is at a bridge between doing most things on her own, but not managing her medication, phone calls, etc. Again, if your parent needs more hands on, this is great.

The tour guide was very friendly and knowledgeable. There were some activities going on during our tour. I really liked that the staff and residents seemed to bond very well. The community offers transportation if needed. Wonderfully clean and organized.

My aunt is doing very well here at this community, it was a hard transition. We are overall pleased with the care they are providing her with. The staff is great, they have been very kind and responsive. She is getting involved in the community and trying some of the activities they offer. She has gone on some great outings. The food is good, they do serve well balanced meals. They do a very good job with the cleanliness. This community is a perfect fit for our family. I would highly recommend it.

The facility that Mom was in was nice but too costly so we asked A Place for Mom to help us find one more reasonable. Autumn Leaves fit the bill. A few of my sisters checked out the home and said that it was very nice. Clean, colorful and had friendly people. Mom loves people. Everyone was very helpful in the transition of moving her in a very speedy manner. She seems very happy with her new home.

We did look at a few communities and I know we made the right decision to go with Autumn Leaves as it was the highest of the pack. The community offers memory care and although a lot of the staff is young they are well trained and often change jobs within the community to keep them from getting bored. My husband has been doing better since moving in and is happy with the food and most of the staff. They offer many activities and although he is not able to participate in the activities that require concentration there are other ones he does participate in. The new activities director is great and the residents respond well to her. I would recommend this community to anyone looking for memory care.
